Customized Assessments

Assess the client's medical, functional, cognitive, emotional, and social status to identify problems, make recommendations, and assist in implementing a plan of care

Home Visits and Wellness Checks

In-person visits (home and facility settings) to oversee client's care, monitor overall status, report findings to family, and serve as advocate and liaison
